A Safe Place Sherwood Park has been trying a new technique in dealing with clients using 2 case managers and 1 team Lead. All three are assigned clients and manage their cases from admission through transition back into the community (Edmonton and area, but not including Strathcona County. Strathcona County residents are assisted by an Outreach Worker). Under this service delivery model, the case managers ensure client needs are met, from housing, legal, safety, financial, etc by liaising with other specialized staff (Victim Advocate, Housing Coordinator, Safety Consultant,etc) and outside agencies. Part of the desired outcome is improved success in moving clients forward and shortening their stay in an emergency shelter. The case management team members continue to work with the clients after discharge from the shelter to ensure a smoother transition.
